A Change The World care coordinator serves as the point of contact for the family having work done to their home during the Change The World Weekend event May 18-20. Change The World Allen is needing a few more care coordinators volunteers to serve our community this year.
A Care Coordinator is someone who is friendly, caring, and is a good listener. Care coordinators have a heart for people and are willing to be helpful to those who have physical or emotional needs.
The duties of a care coordinator include:
- Make initial contact with the family. This contact will be scheduled by the CTW team and project scope team. At this meeting, the care coordinator will be introduced to the family during a brief visit.
- Deliver worker lunches and visit with the family on project work day. The care coordinator picks up lunches for the work team at FUMC Allen. During this second visit, the care coordinator will complete a needs checklist with the family.
- Conduct a follow up visit with the family. After the work has been completed, the care coordinator sets up a third visit to check in with the family and to complete a three-question survey.
